Medication Management
We have Colorado State Certified QMAPs that provide medication administration. We use an online medication management and observation charting program that offers medicaiton reminders and state approved charting management.
2 and 4-Hour Wellness Checks
Our dedicated staff have eyes on residents nearly every minute due to our wide open spaces and cozy environment. Staff also checks on those who are not in constant view every two hours during the day and every four to six hours during the night, depending on their need and care plan.
Community involvment, Outings and Regular Activities
We use person centered thinking in assisting residents with creating the monthly activity calendar. We help them find ways to be part our neighborhood and get involved with the community and other Seniors. When encourage residents to get out and about, keep doing things they enjoy and help them do it!
Fall and Balance Assessment
Through a thorough initial pre-assessment, conversations with primary care providers, family members and other care givers we are able to assess balance and fall risk issues. We are able to get further assessments with the help of phycial and occupational therapists to help ensure best balance practices and create fall prevention plans that are carried out by our dedicated staff.
Geriatric Care Management
Through continuity of care with primary care providers, our staff and care management plans allow us to keep track of progress and needs. With weekly, monthly and yearly care plan meetings that involve staff, providers, residents and family members we are abe to effectively provide exellent care as our residents continue to age. We have partnered with Lykos Medical for in house primary/geriactric care, where primary care comes us for all our medical needs!
